*Program Details:
1. A new patient is defined as the following:
- Acupuncture: New Patient exam, 1st and 2nd treatments are completed. (Completed in 2 visits)
- Chiropractic: New Patient exam, Report of Findings and follow-up adjustment are completed. (Completed in 3 visits)
- Massage: 2 massage appointments of at least 60 minutes have been completed. (Completed in 2 visits)
- SHAPE ReClaimed: New Patient exam, Report of Findings and 1st follow-up are completed. (Completed in 3 visits)
2. Worker's Comp and Personal Injury patients are not eligible to receive a discount on services.
3. Medicare patients are not eligible to receive a discount on services. However, Medicare patients are allowed to transfer their referral massage to an eligible friend/family member.
